Primary Care Physician - Fresno & La Quinta, CA (2 positions available)
This role is mostly onsite but could have some in-home visits. The hours are mostly
8-5 with the ability to work mobile a few days a week. No weekends at this time.

Client, a health services provider, is seeking a Primary Care Physician to join their team. This role involves performing comprehensive in-person assessments and collaborating with the Interdisciplinary Team (IDT) to develop care plans for
participants. Under the guidance of the Medical Director, the Primary Care Physician will manage participant care in various settings, including nursing homes and hospitals, overseeing admissions and discharges across acute and long-term care facilities. Additionally, the role involves coordinating with medical specialists to ensure seamless care coordination and providing leadership to Nurse Practitioners and other IDT members.

• Conduct thorough assessments and provide ongoing primary care to participants.
• Collaborate with external physicians and IDT for effective discharge planning from acute or long-term care settings.
• Actively engage in IDT meetings to communicate participant changes, collaborate on care plans, and coordinate comprehensive 24-hour care.
• Regularly visit nursing home participants to manage and oversee care.
• Participate in a rotation for night and weekend on-call duties.

Job Requirements:

• Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
• 5 plus years of experience
• Doctorate Degree in Medicine.
• Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred).
• Active California Physician License without restrictions.
• Valid state CDS Certification and DEA registration.
• Eligibility or current enrollment.
